Medical Record Report

The Medical Record Report provides a readily available extract from the patient medical record. You can run the Medical Record Report for any patient in the facility or facilities you have access to.

Use this report to create a quick abstract for:

  • chart review (which in some cases may circumvent the need to pull the chart at all)
  • physician peer review committees
  • detailed research of patient care issues

This report includes:

  • patient demographic information
  • financial information
  • clinical information
  • physician information

Prompts

There are only two prompts for the Medical Record Report and both are required:

  1. Facilities
  2. Patient ID or Medical Record:
    • You must have the exact patient IDs or the exact medical record numbers of the patients.
    • You can type in the patient ID or medical record number or import a text file (.txt) that contains those numbers, separated by semi-colons. The import limit is 1,000 patient IDs.
    • To run a medical record report for several patients, change Equals to In in the Selected pane, then separate medical record numbers or patient IDs with a semi-colon as follows:

Note: If a patient is an outpatient, a dash and four digits are added to the end of their Patient ID. These four digits indicate the discharge date of the outpatient. For example, if a patient’s ID is 123456-1206, this is an outpatient that was discharged on 12-06.

Analysis Sample

A Medical Record dropdown list appears at the top of the analysis that contains all the medical record numbers of the patients retrieved. Use this dropdown list to toggle between medical records. Only one medical record appears on the screen at a time.

Printing the Medical Record Report

Click the print button. The Expand Page-by popup window appears.

  • Select Expand Page-by if you want to print all the medical records in the Medical Record dropdown list in one PDF. Each record is a chapter with its own bookmark in the left panel of the PDF.
  • Clear Expand Page-by if you want to print only the current medical record on the screen
