CMS Readmissions and Mortality

The 2024 Top Performers by Focused Population based on the CMS Readmission and Mortality methodologies were calculated using the CMS FY25 focused population definitions.

Important Reminder:Starting with the September 30, 2019 release, the CMS Stroke Readmission population was retired, while the CMS Stroke Mortality population remains active. In order to maintain reporting consistency, QualityAdvisor now maintains both a Current and an Historical version of the CMS defined focused populations.

Beginning with the 2019 Top Performer peer groups and going forward, the CMS Readmission and Mortality peer groups are based on the Current version of the Focused Population definitions.
